We are currently searching for an enthusiastic and values-oriented Case Manager. In this position, you will offer case management assistance to individuals in the Open House Program. Collaborating closely with clients, participants, or residents, you will formulate and execute case plans to realize self-determined objectives.

This position actively contributes to the professional execution and delivery of supported accommodation and psychosocial support services, catering to clients with varying degrees of mental health concerns, ranging from low to mild, as well as episodes of intensive mental health needs.

Key Responsibilities:

  • Support Client Goals:
    • Provide support and encouragement for clients to achieve their individual goals within established case plans.
    • Accompanied clients to appointments and advocated on their behalf with other service providers.
  • Intake and Assessment:
    • Conduct intake and assessment sessions with clients to develop a mutual understanding of their situations, and identify problems, strengths, and needs.
  • Record Maintenance:
    • Maintain accurate and up-to-date client records and data, adhering to TSA standards and procedures, utilizing TSA’s specified electronic database.
  • Preventative Measures:
    • Take appropriate preventative action in response to potential high-risk situations, in accordance with established policy and procedure.
  • Advocacy:
    • Advocate on behalf of clients by communicating with various government and non-government agencies, both verbally and in writing.
  • Establish Effective Relationships:
    • Foster effective relationships with clients that promote self-determination and active participation in their support journey.

This is a permanent full-time position, located in Darwin, NT. Salary and conditions are in accordance with the SCHADS Level 4 Award.

Your Skills and Experience:

  • Preferred Degree Qualification:
    • Possess a degree in Social Work, Mental Health, Welfare, or Community Development, coupled with relevant fieldwork experience.
    • Demonstrate a high level of knowledge in the sector, acquired through prior appointments, service, and educational development.
  • Alternatively, Minimum Certificate IV Qualification:
    • Hold at least a Certificate IV in Community Services in Social Work, Welfare, or Community Development.
    • Showcase high-level work experience, sector knowledge, and skills gained through similar appointments, service, and study/development.
  • Experience and Skills:
    • Demonstrate experience in engaging clients within the Justice System or Corrective Services.
    • Exhibit a willingness to provide support in areas such as homelessness, alcohol and other drug dependence, and mental health issues.
  • Group Facilitation:
    • Showcase experience in or a willingness to learn skills related to the co-facilitation of groups.
  • Required Checks:
    • Complete a National Police Record check before commencement.
    • Obtain an NT Working with Children Ochre Card check before starting.
  • Driver’s License:
    • Hold a current NT driver’s license.
  • Eligibility to Work:
    • Provide proof of eligibility to work in Australia.
